Sickle Cell Information
Effective in the 2022-23 academic year, the NCAA required all Division III student-athletes provide confirmation of their sickle cell trait status (SCT), either through 1) existing documentation from birth, or 2) recent screening prior to any intercollegiate athletic participation.
All new and returning student-athletes should read through the information included in the Sickle Cell Packet. After reading through the packet, please fill out the Sickle Cell Status Reporting Form linked both
here and on page three of the Sickle Cell Packet.
Free Physicals are available through St. Luke's June 24th and July 15th at Coca Cola Park or through Alvernia Health and Wellness on June 23rd at Alvernia's Medical and Counseling Center. To register for St. Luke's, click

As a student at Alvernia University, you are required to have health information on file with the University's Health & Wellness Center. You will complete these requirements on the Student Health Portal. Please log into your Student Health Portal using your Alvernia Network ID. Be sure to log into Alvernia's Office 365 before accessing the Student Health Portal to reset your initial password to a personalized password.
Once you access your Student Health Portal. Complete the Sports Medicine Compliance Form in 'My Forms' and upload the required information on the forms or through the 'Document Upload' feature. If you do not have a copy of your immunization record, check with your high school nurse or your medical provider to obtain a copy. You will need to enter this information and upload a copy to us.
